Changed Lives Mental Health and Substance Abuse Counseling LLC is a privately owned company dedicated to providing chemical dependency services with qualified staff. The utilization of highly qualified professionals ensures the delivery of services in an effective and efficient manner with optimum results. Changed Lives Mental Health and Substance Abuse Counseling LLC strives to employ and/or contract with individuals of good character who are capable, qualified, and willing to carry out the philosophy of Changed Lives Mental Health and Substance Abuse Counseling LLC in delivering services to clients and the community. The following policies and procedures are applicable to all employees of Changed Lives Mental Health and Substance Abuse Counseling LLC regardless of their position.

• The administrator, clinician, and staff will be subject to a criminal background check from the Kentucky State Police.
  
• Changed Lives Mental Health and Substance Abuse Counseling LLC will not employ any individual who has been released from probation or parole for the conviction of a violent crime, hate crime, or sex crime within two (4) years from the date of his or her crime.
  
• Changed Lives Mental Health and Substance Abuse Counseling LLC staff, contractors, practicum students, and volunteers must be knowledgeable of and comply with, state and federal confidentiality laws.
  
• Changed Lives Mental Health and Substance Abuse Counseling LLC will provide a written job description for each position stating qualifications, duties, and reporting supervisor.
  
• Changed Lives Mental Health and Substance Abuse Counseling LLC will ensure that all personnel is qualified for their position through documentation of education, work experience, and professional licensure, certification, or registration.
  
• An individual who has had a conviction for the neglect, physical abuse, sexual abuse, or sexual exploitation of a child will not be employed by Changed Lives Mental Health and Substance Abuse Counseling LLC.

Changed Lives Mental Health and Substance Abuse Counseling LLC is committed to:

• Contributing to the development of each profession by supporting its standards and by encouraging meaningful, progressive change.
• To provide effective care in an outpatient setting for individuals in need of treatment for chemical dependency.
• To provide families, significant others, and the community with the effective treatment resource for individuals experiencing problems with chemical dependency.

Changed Lives Mental Health and Substance Abuse Counseling LLC will not accept a client where a private relationship exists that will result in a conflict between the program’s interests and the interests of the client, or a situation will be created where Changed Lives Mental Health and Substance Abuse Counseling LLC personal or financial interest conflicts with professional responsibility.

Changed Lives Mental Health and Substance Abuse Counseling LLC certified, clinical or administrative staff shall not be currently employed as:

• A law enforcement officer
• A corrections officer
• A probation officer
• An attorney
• An employee of the Administrative Office of the Courts
• An employee of The Division of Substance Abuse
• A judge

Changed Lives Mental Health and Substance Abuse Counseling LLC employees will not accept a spouse, child, stepchild, sibling, parent, stepparent, grandparent, grandchild, aunt, uncle, niece or nephew as a client, unless there is no conflict of interest between instructor and client. Family members will be referred to another appropriate provider, whenever possible.
